HardCover Products Inc. Institutional Price Lists Request Form

In order to be eligible to receive our Institutional Price Lists, you must be making the request on behalf of one of the following types of organization:

• Government Agency, Federal, Provincial or Municipal
• Public Health Agency
• Community Health Centre
• AIDS Committee, or Outreach Program
• School Health Centre - University, College or High School

If you belong to another type of organization that is not currently listed above but, you feel your group should be included, kindly complete the form and provide additional information in the allotted space at the end of the form.

If you are an individual interested in purchasing our condom or lubegel products and meeting or exceeding our minimum quantity order of 3 gross of any combination of condoms or lubegel, you must still complete the form. Kindly identify the products you would like to purchase and we will reply with a quote for the cost including shipping and taxes. Please note, payment must be by VISA or Mastercard only and must be confirmed by a telephone call. DO NOT INCLUDE YOUR VISA OR MASTERCARD INFORMATION IN YOUR INITIAL EMAIL REQUEST. You will be asked to supply the information once you have accepted our quote for the items you have selected.

Requests for price lists are typically handled within 1-2 business days.
