Request For HardCover Price List

In order to to receive our Institutional Price List, you must be making a request on behalf of one of the following types of organizations:

Government Agency, Federal, Provincial or Municipal
Public Health Agency
Community Health Centre
AIDS Committee, or Outreach Program
School Health Centre - University, College or High School

If you are affiliated with an organization that is not listed above, and would like to receive our price list, kindly complete the Request Form and provide the required information.

If you are an individual interested in purchasing our condom or lubegel products in the minimum required quantity of 3 gross of any combination of condoms and/or lubegel, you must complete the Request Form. Please indicate the products you would like to purchase and we will provide a quote for the order as well as shipping and taxes. Payment must be by VISA or Mastercard only, and must be confirmed by telephone. DO NOT INCLUDE YOUR VISA OR MASTERCARD INFORMATION IN YOUR INITIAL EMAIL REQUEST. You will be asked to supply the information once you have accepted the quote for your order.
